What to Bring on the day of surgery.
 

  • Driver’s license & all medical insurance cards

  • Diabetic medications (insulin/supplies/oral meds)

  • Inhalers

  • Medication – if requested in your pre-surgical call

  • CPAP machine, if you have one

  • Any medical equipment - including oxygen tank or compressor with cord if you are on home oxygen

  • Paperwork received from your surgeon’s office

  • Family member or friend to wait for you and to transport you home.

 

 

Persons under 18 must have a parent or legal guardian with them to sign the consent forms.
